Claim Guidelines
Using Our “Medical Card”
In case of an accident, the Insured should present our “Medical Card” to the network hospitals that are administrated by the Ministry of Health of PRC for immediate treatment.
TheChina Pacific Insurance Co., (H.K.) Ltd. will verify the supporting documents that are submitted by hospitals and reimburse the medical expenses to them directly.
Not Using Our “Medical Card”
In case of an accident, if the Insured does not present our Medical Card to hospitals and pay the medical expenses himself, the Insured should submit the following supporting documents to China Pacific Insurance Co., (H.K.) Ltd. directly:
1) Claim Form
(To be completed by the Insured)

2) Original Medical Certificate that states the Insured’s nature and extent of injuries, diagnosis and treatments
(To be issued by the Attending Doctor)
(To be issued by the Attending Doctor)
3) All Original bills/receipts with detail breakdown of costs/expenses
(To be issued by the Hospitals/Clinics)
(To be issued by the Hospitals/Clinics)
4) Original report that verify the alleged accident
(To be advised by the Official Authorities)
(To be advised by the Official Authorities)
Claim Form could be
• downloaded here
• obtained by fax via telephone number (852)2137 7639 of the Claims Department
• acquired in person at the office of China Pacific Insurance Co., (H.K.) Ltd.
